Stephanie Cesar is a compassionate and dedicated Registered Nurse with extensive experience in case management, patient advocacy, and delivering high-quality care to the aging population. With a strong foundation in nursing and health studies, Stephanie has built a career focused on improving patient outcomes, particularly in the areas of behavioral health and home health services.

Stephanie served as an RN Case Manager at South Shore Hospital’s Care Progression Department, where she successfully managed a diverse caseload and coordinated multidisciplinary care to enhance patient outcomes. Her role involved developing and implementing personalized care plans, working collaboratively with interdisciplinary teams, and educating patients and families about treatment options. Prior to this, she worked in behavioral health and home health settings, where she provided in-home care, mental health stabilization, and advocacy for vulnerable individuals and families.

Stephanie’s previous experience includes nursing roles at Charlton Memorial Hospital, where she provided exceptional care in a fast-paced medical-surgical telemetry unit, as well as earlier work with Bethany at Home, where she supported patients with daily living activities in a compassionate home care setting. She also served as a Family Support Advocate at Health Imperatives, helping connect families with essential community resources.

Known for her strong communication, organizational, and patient-centered care skills, Stephanie is committed to expanding her nursing expertise and advancing in the field of Care Management to continue delivering exceptional patient care.
