Emma Fortin is a highly experienced LICSW (Licensed Independent Clinical Social Worker) with over 15 years of expertise in working with older adults and individuals with disabilities across a variety of community and healthcare settings. She has built a notable career, having served in several impactful roles, including as the Community Discharge Planning Specialist Supervisor at Springwell, Inc. In this position, Emma specialized in managing complex psychosocial and medical discharges for older adults and individuals with disabilities transitioning from inpatient settings to community living. As part of this initiative, she oversaw the implementation of several MassHealth based programs on her clients’ behalf, including the Frail Elder Waiver (FEW), the Personal Care attendant Program (PCA) and Money Follows the Person Program.
Her career also includes serving as a Clinical Consultant for Social Work in Progress, where she provided clinical supervision and consultation in skilled nursing facilities, ensuring the delivery of high-quality care. Emma has also held the position of Director of Social Services at Wingate at Andover, a skilled nursing facility, where she oversaw social work services and patient care initiatives.
Emma holds the certificate of Care Manager Certified (CMC), demonstrating her commitment to ongoing professional development. She is also a member of the Aging Life Care Association at the Advanced Professional Level, reflecting her expertise in aging services. Emma currently serves on the Board of the Aging Life Care Association New England Chapter. She holds a Master’s degree in Social Work from Boston College, with a clinical concentration in Older Adult & Families, equipping her with a deep understanding of the unique challenges faced by aging individuals and their families. Emma’s career is a testament to her dedication to improving the lives of those in her care, with a particular focus on advocating for older adults and individuals with disabilities.
